Wakayama, Japan - In one of the first studies to use optical coherence tomography (OCT) imaging to compare plaque vulnerability in different groups of at-risk subjects, Dr Takashi Kubo (Wakayama Medical University, Japan) and colleagues say they can identify important differences in the plaques of AMI patients and of patients with stable angina [1]. Investigators also found that plaques that met the definition of thin-cap fibroatheroma (TCFA), as well as thinner fibrous caps generally, were more common in the AMI patients than in the stable-angina subjects and that this was true not just of the infarct-related lesions, but also of the noninfarct lesions."

Pacienti s diabetes mellitus 2. typu majú zvýšené riziko rozvoja kardiovaskulárnych ochorení už v čase stanovenia diagnózy diabetu. V súbore 1471 novodiagnostikovaných diabetikov 2. typu sme analyzovali spektrum rizikových faktorov aterosklerózy. Priemerné hodnoty glykovaného hemoglobínu (GHb) pri stanovení diagnózy diabetu boli 8,4 %, priemerné hladiny LDL cholesterolu boli 3,4 mmol/l a priemerné hodnoty krvného tlaku boli 140/84 mmHg. V čase stanovenia diagnózy cukrovky malo 74 % diabetikov hladinu GHb >7,5 %, 87 % diabetikov malo hladinu LDL cholesterolu > 2,5 mmol/l a 64 % diabetikov malo hodnoty systolického tlaku krvi (STK) > 130 mmHg. Za použitia kalkulátora rizika UKPDS Risk Engine bolo vypočítané absolútne 10-ročné riziko rozvoja ICHS u mužov s novozisteným diabetom 23,1 %, zatiaľ čo u žien bolo toto riziko 14,4 %. Ak by boli u pacientov dosiahnuté cieľové hladiny GHb, LDL cholesterolu a STK, potom by sa vypočítané riziko rozvoja ICHS u oboch pohlaví znížilo o vyše 40 % a riziko kardiovaskulárnej mortality by sa znížilo až na polovicu. Uvedené fakty svedčia a vysokej rizikovosti pacientov s novodiagnostikovaným diabetom 2. typu a o vysokom preventívnom potenciáli intervencií zameraných na liečbu hyperglykémie, hypercholesterolémie a hypertenzie. RISK PROFILE AND ANTICIPATED BENEFIT OF PHARMACOLOGICAL TREATMENT IN PATIENTS WITH NEWLY DIAGNOSED TYPE 2 DIABETES

The patients with type 2 diabetes have an increased risk of cardiovascular disease even in the time when the diagnosis of diabetes is established. We have analysed atherosclerosis risk factor profile in a group of 1471 newly diagnosed patients with type 2 diabetes. Mean glycated haemoglobin (GHb) at the time of diagnosis was 8,4 %, mean LDL cholesterol level was 3,4 mmol/l, and mean blood pressure was 140/84 mmHg. At the time of diagnosis of diabetes 74 % of diabetic patients had GHb level >7,5 %, 87 % of patients had LDL cholesterol level > 2,5 mmol/l, and 64 % of diabetic patients had systolic blood pressure above 130 mmHg. Using the UKPDS Risk Engine calculated 10-year absolute risk of coronary heart disease in men with newly diagnosed type 2 diabetes is 23,1 %, while in women it is 14,4 %. If target GHb, LDL cholesterol and systolic blood pressure values were achieved, then calculated risk of coronary disease in both genders would decrease by more than 40 % and the risk of fatal cardiovascular disease would be reduced by one half. These facts indicate high risk of patients with newly diagnosed type 2 diabetes and show also high preventive potential of treatments focused on high blood glucose, cholesterol and blood pressure levels. Someone asked me today about how to analyze a two group experiment with a baseline value. This is common research design. Researchers will assess all patients at the beginning of the study.
